What Information Do We Collect?

The data we collect helps us provide you a meaningful and customized experience while using our Services, and they also help you and the Appropriate Medical Staff to better manage your health. We collect two categories of information from users: (1) Personal Information and (2) General Information.

You may also choose to input certain health information, including but not limited to weight, height, heart rate, systolic, and diastolic blood pressure, glucose level, you level of stress, medication, laboratory results, familial history and activity. When you synchronize a point of care (“Device”) with the App, we collect the following information from your Device, which tracks detailed physical and health information based on monitoring your activity and your health status as set out in this Privacy Policy. This information may include your heart rate, systolic and diastolic, ECG, weight, glucose level, level of stress, and level of activity. This data is translated into information such as your trends and progress. Your Device stores this data until you sync. We may also collect your precise location data when you use our mobile app along with other data that we normally collect through our mobile applications.

We only collect Personal Information you voluntarily provide to us, including through registrations, order information, applications, surveys or other inquiries. We do not collect information that would identify you unless you choose to provide it to us. You can choose not to provide us with certain information, but please be aware that may impact the Services provided to you.

“General Information” is information we obtain automatically from your connection, interaction with this App and the use of Cookies and other technologies. General information does not identify you personally. General information we collect when you use our App may include but is not limited to the domain and host from which you access the Internet, the Internet protocol address of the computer or smartphone you are using (“IP Address”), the browser software you are using, device type, manufacturer, model, and operating system you are using, the date and time that you access our App, the specific pages or features you use, and the number of visits you make to our App.

“Cookies” are small pieces of information which the App or the website stores on your computer hard drive and can later retrieve to recognize your browser and track your use for marketing purposes. On behalf of the website provider Google will use this information for the purpose of evaluating your use of the website, compiling reports on website activity for website operators and providing other services relating to website activity and internet usage to the website provider. “Localisation Data” WHEN YOU INSTALL THE APP, YOU WILL BE ASKED TO GRANT ACCESS TO YOUR MOBILE DEVICE’S GEOLOCATION DATA. By granting such permission, we may collect information about your precise geolocation information (i.e., your real-time geographic location), and may use that information to customize the Service with location-based information and features (for example, to provide you with relevant weather information through your weather app based on your location) and allow the Appropriate Medical Staff to will help directly or indirectly to manage your health.

“Personal Health Information” means identifying information about an individual in oral or recorded form, if the information,

“Personal Information” means Information about the individual that relates to their person. Personal information includes Personal Health Information, and an individual’s contact information such as home address, age, gender, identifiers such as driver’s license, health number, social insurance number and information such as an individual’s racial or ethnic origin, political opinions or religious beliefs.

“Pixel Tags” We may use spotlight ads/web beacons/GIF/pixel tags which are site instrumentation tools that help us to determine, for instance, whether a page has been viewed or not and, if so, how many times. Emails or electronic newsletters we send may use tools (e.g., pixel tags or web beacons) to gather email metrics and information to improve the reader's experience such as how many of the emails are opened, if they were printed or forwarded, the type of device (e.g., mobile or PC) from which they were opened, and the city, state, and county associated with the applicable IP address. In general, any electronic image viewed as part of a webpage, including an ad banner, can act as a web beacon.

“Log Files” When you visit our website, the server housing the pages automatically generates a “session log.” We use session logs to help us determine how people travel through our Site. In this way, we can structure our pages so that the information most frequently visited is easier to find. By tracking page visits, we can also determine if the information we're providing is being used. The only data gathered is the Internet Protocol (IP) address from which you came (which contains no personal information), the web site that referred you, the pages you visited and the date and time of those visits.

“Substitute Decision Maker” means a specified person(s), identified as per the hierarchy of persons set out in the Health Care Consent Act, 1996, who may make a treatment decision for someone who is mentally incapable of making his/her own decisions.

We may occasionally contact you to gather customer service information to help us determine how we can improve our Services to better meet your needs. Because we provide Services that supports the coordination or management to health, rehabilitation, wellness and related services, we collect the sort of Personal Information needed in order to provide such Services. We may also use De-Identified Information and/or aggregate data about users of the App for various business, scientific or research purposes including product, service or program development and improvement, scientific publications, marketing supports, performance reports.

We may combine or cross-reference your Personal Information with other information we acquire about you directly or through third party sources, (such as E.g.: Apple, Google…) to help customize the information, Services we provide to you. By accepting this Privacy Policy, you allow us to combine and cross-reference your information.

We may use tools to collect information about use of this App, including but not limited to, Google Analytics or other similar services. Such tools collect information regarding how users use the App, what information they view when they do so, and what other sites and/or apps they used prior to coming to this App. We use the information we get from such tools only to improve our Services, tailor content, deliver a better experience and manage our advertising. Google’s use of information it acquires about you or your use of Greybox is governed by Google Analytics Terms of Use and the Google Privacy Policy.

We may also collect, aggregate and use anonymized data about the visitors to our App for analysis and share such aggregate anonymized information with advertisers, business partners, sponsors and other third parties.

With respect to the collection and use of General Information, you have the ability to disable or manage the use of cookies on your computer using controls in your browser. However, you are not able to opt out of the uses of General Information otherwise collected as set forth in this policy. Please note that certain features may not be available when cookies are disabled. Also, please be aware that at this time, we are not honoring “do not track” signals from Web browsers. With Whom Do We Share Your Information?

Except as set forth in this Privacy Policy or as specifically agreed to by you, we will not sell or rent your Personal Information to third parties or disclose any Personal Information we gather from you on through our App to third parties.

When you register to the Services provided by Greybox, all the Appropriate Medical Staff will have access to your Personal Information. In addition, only the employees at Greybox who need to access your Personal Information to fulfil their employment duties will have access to it. We may share aggregate De-Identified Information, not any of your personally identifiable information, with our employees. Greybox does not share, sell, or rent personally identifiable information with independent companies for their own use without providing you a choice other than those stated below.

We may share Personal Information about you with partners with whom you’ve given us permission to, such as partner applications and integrations. In addition, by downloading other apps that sync information with the Devices, you expressly consent to providing your Personal Information with the company that develops that app. To combine wearable expertise with technology and design expertise from world-class partners, Greybox works with partners to build and bring you unique wearable experiences. With our partners in connection with specifically branded Greybox products, we may share Personal Information according to the paragraph below. To the extent you publicly share your Personal Information, the restrictions of this Privacy Policy do not apply to such Personal Information. Once displayed on publicly viewable web pages, that information can be collected and used by others. We cannot control who reads your posting or what other users may do with the information that you voluntarily post. Once you have posted information publicly, while you will still be able to edit and delete it on the Site, you will not be able to edit or delete such information cached, collected, and stored elsewhere by others (e.g., search engines).

From time to time, we may use Third Parties to provide Services or otherwise support our business or collaborate with third parties with respect to development, promotion or other business activities related to a particular product or service. As a result, we may disclose Personal Information to contractors, service providers and other third parties but such disclosure will be limited to enabling those third parties to provide their products or services. To the extent a third party’s Privacy Policy separately governs their use of your data obtained via Greybox, you will be notified and given an opportunity to review such terms. We may also disclose Personal Information to our subsidiaries and affiliates.

We may share information with a parent company, subsidiaries, affiliates, joint ventures, or other companies under common control with us. In the event of a change of ownership, sale, merger, liquidation, reorganization or acquisition of Greybox, in whole or in part, your information may be transferred as part of the transaction, including during the due diligence process, as long as, the party acquiring such information agrees to be bound by the terms of this Privacy Policy.

We may also release your Personal Information to third parties as required by law, when we believe disclosure is necessary to comply with a legal or regulatory requirement, judicial proceeding, court order or legal process served on us, to protect the safety, rights or property of patients, customers, the public or Greybox or defend Greybox and its officers, directors, employees, attorneys, agents, contractors and partners, in connection with any legal action, claim, or dispute.

How We Keep Your Information Secure

Your Personal Information is stored on servers hosted in Canada, the United States or Europe depending upon your geographic location. We may add a server location in other regions depending upon business need. Local tribunal and legislation will have jurisdiction with regards to the access to your Personal Information.

We seek to safeguard the security of your Personal Information and have implemented reasonable security measures consistent with applicable laws, safe harbors, and accepted practices in the healthcare industry to protect the confidentiality of your Personal Information. We have put in place a variety of information security measures, policies and procedures to protect your Personal Information, including “Secure Socket Layers” or SSL encryption technology, to protect your Personal Information during data transport and at rest. We limit the access to your Personal Information to employees or partners of Greybox who needs it for the execution of their duties and /or provision of Services. Despite our efforts, however, we cannot guarantee the absolute security of your Personal Information, nor can we guarantee that information that you provide will not be intercepted while being transmitted to us over the Internet. There is always some risk that an unauthorized third party may find a way around our security systems or that transmissions of your Personal Information over the Internet will be intercepted. Therefore, we urge you to also take every precaution to protect your Personal Information when you are on the Internet or using the App. In order to protect your privacy, never share your sign-in name or password and always log out of the App when you are finished using the service.

We will store and maintain your Personal Information in accordance with this Privacy Policy and as required to provide you Services through the App. When you cease using the Services or if your account is inactive, your Personal Information will be kept from the last data recorded for a period necessary to comply with our legal obligations, resolve disputes, and enforce our agreements.

Right To Access And Correct

On written request and subject to proof of identity, you may consult your Personal Information that we have collected, and ask that any necessary corrections be made, where applicable, as authorized or required by law.

However, to make sure that the Personal Information we maintain about you is accurate and up to date, please inform us immediately of any change in your Personal Information with the information at the bottom of this Privacy Policy.

You may opt-out of providing your Personal Information to the Services at any time by uninstalling the App. You can opt-out of promotion emails from us by following the unsubscribe instructions provided in the e-mail you receive or by contacting us directly (please see contact information below). If you opt-out, we may still send you non-marketing emails including emails about your accounts and our business dealings with you. You may send requests about Personal Information to our contact information below. You can request to change contact choices and marketing choices and to update, review, delete or change your Personal Information. We will use commercially reasonable efforts to honor your request. We may retain an archived copy of your records as required by law or for legitimate business purposes.

At Greybox, we are dedicated to providing you with transparency as to how we use your Personal Information. If you remove data from your Greybox Account or from the Greybox Service, it will no longer appear to you or others who use the Site or Service. IF YOU WOULD LIKE TO DEACTIVATE YOUR ACCOUNT ON THE SITE OR SERVICE OR UNSUBSCRIBE TO OUR PROMOTION MAILING LIST, PLEASE CONTACT OUR CUSTOMER SERVICE DEPARTMENT AT support@greybox.biz . When you deactivate your account on the site or service, data that can identify you will be removed from the Site or Service. Greybox may continue to use your de-identified data after you deactivate your account.

Update

Our Privacy Policy may be periodically amended without notice to reflect changes made to our practices. We therefore invite you to consult this Privacy Policy in regularly for any changes. Continued use of our Services after the effective date will be construed as an acceptance of these amendments on your part. The date of the last update is the one that appears at the top of this Privacy Policy.

Other Important Notices

Children’s Privacy: We are committed to protecting the privacy of children. As such, we do not support accounts for people under eighteen years (18) of age.

Third Party Websites and Ad Services: Please be aware that our App may have links to third party websites that may collect Personal Information about you. When you click on one of these third party links, you are entering another website. We do not have control over any third party website and this Privacy Policy does not cover the information practices or policies of such third party websites. We encourage you to read the privacy policies of all such websites since their privacy policies may be materially different than our Privacy Policy.

In addition, some advertisements on the App are served by third party advertisers, ad networks, ad servers and/or via Google. These third parties may use cookies alone or in conjunction with web beacons or other tracking technologies to collect information about our users. This may include information about users' behavior on this App and other websites to serve them interested-based (behavioral) advertising. We do not control these third parties' tracking technologies or how they may be used. If you have any questions about an advertisement, we recommend that you contact the responsible advertiser directly.

E-Mail communications: Do not send e-mail communications containing Personal Information to us. We cannot secure any Personal Information sent by e-mail because such information can be accessed by other internet users. If you send us a question by e-mail, our use and disclosure of that information will be limited to the minimum necessary to respond to your question.

If you provide us feedback or contact us via e-mail, we will collect your name and e-mail address, as well as any other content included in the e-mail. We also collect certain requested information if you contact us via e-mail regarding support for the Product or Services. When you participate in one of our surveys, we may collect additional personal information. De-Identified Information: On occasion, we may make arrangements with certain customers or business partners to share certain de-identified aggregate pattern information in order to assist such customers or business partners to improve their service (such as evaluating patterns, utilization, usage and trends). Aggregate information is information that describes the habits, usage patterns and/or demographics of users as a group but does not reveal the identity of particular users. Your anonymous data is combined with the anonymous data of other users and becomes statistics. We may use aggregate information to understand the needs of our user community and determine what kinds of programs and services we can offer to you. We could use this anonymous information to give potential users or business partners a picture of our community and services. Aggregate information may be provided or sold to third parties. Absolutely no personal identifying information is included in the aggregate reports; each individual remains anonymous.
