The General Information we collect when you use the Application may include, but is not limited to, the domain and network through which you access the Internet, the Internet Protocol (IP) address of the computer or smartphone you are using, the browser software you are using, the type of device, manufacturer, model, and operating system you are using, the date and time you access the Application, the specific pages or features used, and the number of visits made to the Application.
"Cookies" are small pieces of information that the Application or computer stores on your hard drive and can subsequently retrieve to recognize your browser and track your browsing for commercial purposes. To learn more about cookies, visit http://www.allaboutcookies.org. We have the ability to collect information through Cookies.
We may use both session cookies (which expire once the browser is closed) and persistent cookies (which remain on your computer until you delete them) to provide you with a more personal and interactive experience of our Services. You can usually remove and delete Cookies through your browser settings. Many browsers are set to accept Cookies until you change the settings. If you delete or reject our Cookies, it may affect how our Services work for you. We may use a Third Party to provide certain features of our Service, such as filling out an online purchase form from our site. In performing this function, this Third Party may place a persistent cookie on your computer or collect information about you for the purpose of facilitating the purchase process.
"Analytics." The Site may use third-party analytics services (such as New Relic, DART, Omniture tags, DoubleClick, and Google Analytics) to facilitate analysis of how users use the Services. Analytics services use Cookies to collect information such as the frequency of use of the Services for the purpose of improving our Service. The ability of analytics services to use and share the information they collect is restricted by their terms of use and privacy policy, which you should refer to for information on how these entities use such information. We use Google Analytics to analyze the use of our site. This tool may collect certain data, such as your IP address, through cookies. This data may be transmitted and stored by Google on servers located abroad, including in the United States. Google's use of this information is governed by their own terms of use and privacy policy. We invite you to consult them for more information. Our use of Google Analytics complies with all requirements under applicable Canadian privacy laws, including the Personal Information Protection and Electronic Documents Act (PIPEDA). You can refuse the use of Cookies by selecting the appropriate settings on your browser. However, please note that this may prevent the website from functioning properly. In addition, you can prevent Google from collecting data (cookies and IP address) and using it by downloading and installing the browser plug-in available at https://tools.google.com/dlapage/gaoptout. For more information about the terms and conditions of use and data privacy of Google, please visit https://www.google.com/analytics/terms.
"Location data" WHEN YOU INSTALL THE APPLICATION, YOU WILL BE ASKED TO ACCESS YOUR GEOLOCATION DATA ON YOUR MOBILE DEVICE. With this permission, we can collect information about your precise geolocation (e.g., your real-time geographic location) and use this information to personalize our Services with location information and directions (e.g., providing reliable weather data via the weather app based on your location) and enable Designated Medical Personnel to directly or indirectly help you manage your health.
"Personal Health Information" means identifying information about an individual in oral or recorded form, if the information:
- Is related to the individual's mental or physical health, including information about their family's health,
- Is related to the provision of health care for the individual, including the identification of a person as a health care provider for the individual,
- Is a program of services to the individual under the Home Care and Community Services Act, 1994, LO 1994, c 26 on home care and community services,
- Relates to payment and eligibility for health care, or eligibility for health care coverage in respect of the individual,
- Relates to the donation by the individual of any body part or bodily substance, or is derived from tests or examinations of such body parts and bodily substances,

"Personal Information" means any information that relates to a natural person and allows, directly or indirectly, to identify that person. Personal Information includes Personal Health Information and the individual's contact information, such as residential address, age, gender, means of identification such as driver's license, social security number, and information such as the individual's race or ethnicity, political opinions, or religious beliefs.
"Pixel Tags" We use targeted ads/web beacons/GIFs/pixel tags, which are web tools that help us determine, for example, whether a page has been viewed and, if so, how many times. The emails and electronic newsletters we send may use tools (e.g., pixel tags or web beacons) to collect email data and information to improve the user experience, such as the number of times an email has been opened, whether it has been printed or forwarded, the type of device (e.g., computer or mobile) on which it was read, and the state, city, and country associated with the IP address. In general, any image appearing on a website page, including an advertising banner, is likely to be a web beacon.
"History" When you visit our website, the server containing the pages automatically creates a "session history." We use session histories to help us determine how users navigate our Site. This allows us to structure our pages so that the most sought-after information is easier to find. By tracking visits to our pages, we can also determine whether the information presented is useful or not. The only data collected is the IP address (which does not contain any personal information), the referring website, the pages visited, and the time and date of those visits.
« Third Party » refers to any external provider that performs a function related to our Services — for example, to facilitate a transaction or enhance a website feature and that may, in doing so, collect or place data (such as a Cookie) on the user’s device. More specifically, this may include payment service providers, authentication or security providers, cybersecurity and fraud prevention services, analytics or tracking tools, cloud hosting and infrastructure providers, etc.
"Third-party decision maker" refers to the person or persons, identified hierarchically in accordance with the Health Care Consent Act, 1996, S.O. 1996, Chapter 2, who make medical decisions for someone who does not have the mental capacity to make their own decisions.
