The use of the mobile application by people with chronic illnesses makes it possible to adequately monitor specific vital signs and indicators of stability or deterioration in health. Blood sugar, heart rate, blood pressure, blood oxygen, respiratory rate, weight and physical activity reflect your health and variations beyond predetermined thresholds are intended to be an indicator of changes to consider.

The use of connected devices makes it possible to collect data and transmit it to care teams. These data, combined with qualitative and quantitative questionnaires, facilitate the detection of changes in health status and allow action to be taken before serious deterioration.

Dr. Nathalie Saad, pulmonologist and the Mont Sinai Hospital of the CIUSSS West-Central Montreal are the recipients of the 2023 Hippocrates Grand Prize for the pulmonary tele-rehabilitation program, using the TAKECARE platform.

In collaboration with our partner OROT – Connected Health Innovation Hub and the Jewish General Hospital of Montreal, multiple clinics use the TAKECARE application to monitor patients with chronic conditions.
